“Potential anatomical spaces” have attracted surgeons in the past and in recent years. Due to the development of modern imaging techniques and the advent of minimally invasive surgery, access to these spaces has become a real surgical option. The purpose of this book is to document the challenges related to “potential anatomical spaces”, traditionally described as “hidden” spaces. The spread of video-assisted surgery and its application in the management of diseases involving organs or anatomical structures placed in the “potential spaces” (neck, mediastinum, pro-peritoneum and retro-peritoneum, subfacial space of the leg, and axilla), has rendered their surgical anatomy less abstract. Development & Plasticity in Sensory Thalamus & Cortex is based on a 2-day symposium offered for the first time at SFN 2003. The symposium focused on the latest findings and ideas, focusing on visual and somatosensory thalamus and neocortex in rodents and carnivores and functional imaging studies in developing and aging human neocortex, as well as plastic changes after spinal cord injury. This volume will cover recent advances in understanding sensory thalamic and cortical function, organization, and plasticity. Chapters will cover a broad range of approaches from single cell recordings to viral gene transfer to transgenic and knockout mouse models to functional imaging in human cortex. The first of the four procedures is to block and contain either the eruption of anxiety or collapse of depression in a patient, usually through pharmacological intervention. Once the anxiety and/or depression has been temporarily contained, the patient will be able to function through the rest of the therapeutic process. The second procedure is to identify and isolate the step that leads up to the eruption of anxiety or collapse of depression, thus allowing the patient to avoid walking into similar situations that lead to catastrophe. Thirdly, the practitioner works with the patient to help her realize that there is always a step in the opposite direction from the old step, one that used to be suppressed by the oncoming anxiety or depression. The identity of this new step will release the patient from the traps of the old patterns, and will free her to enter into a new trajectory. The fourth procedure is to notice that the old step had locked the patient into a (seemingly) inescapable trap, but that any new step(s) taken will also carry with them the potential for further anxiety and depression. The completion of this final procedure should allow the patient to see that mental health comes from the combined ability to avoid the pitfalls of old steps, while accepting the inherent risks in taking new steps. The success of this brief therapy model is fully realized when a recovered patient begins to fall into a new trap, and can duplicate this process on her own.

Brain metastases are the most dreaded complication of systemic cancer, affecting approximately 170,000 people a year, a far greater incidence than primary brain tumors. Advances in modern chemotherapy have led to an improved control of system cancers, often with agents that poorly penetrate the central nervous system, resulting in an increasing incidence of brain metastases. Traditional chemotherapy has had little impact on brain metastases. The main therapeutic option is radiation therapy, and in a small number of patients - surgery. However, refinements in all of these treatments offer some positive outcomes and an increased ability to stratify patients based on certain criteria.
